Physical Training & Injury Risk

I acknowledge that Martial Arts and fitness training involve intense physical training and physical contact. Whilst every effort is made to ensure that no injury is suffered, I appreciate the possibility that in participating in this activity, I may suffer physical injury.

Notwithstanding, I agree to accept all responsibility for any and all injury I may suffer in participating in Martial Arts and fitness training and further agree, that in consideration of being accepted to train, to release, waive and forever discharge the organization and instructors from any and all claims I may have in relation to participating in classes, training, tournaments, gradings, or other events conducted or staged by or in connection with Eltham Martial Arts Academy and associated facilities, the instructors, or any other person associated with the organization.

Capacity & Guardianship

I make this acknowledgement and disclaimer in my own capacity and as parent, spouse, and/or guardian of those others listed in this application, specifically in respect of Martial Arts and fitness training and classes.

Medical Fitness & Safety

I confirm that I am (and/or any listed minors are) in good physical health and medically fit to participate in martial arts and fitness training. I agree to notify instructors immediately of any injuries, medical conditions, or changes in health status that may affect safe participation.
